What services are you currently receiving or have you received from TEAM EEI? (Example: Computer Training, Job Seeking Services, and Job Coaching etc.)

How would you rate the knowledge of your trainer and overall TEAM EEI staff? This includes all areas of service provision such as hardware and software to access and knowledge of area that you are working with them in.

Is your trainer on time for your appointments? Is your trainer distracted or appear to be preoccupied during your training sessions?

Is your trainer easy to contact as needed? This would include for questions or concerns that you may have in addition to scheduling and other purposes.

Do you feel that your trainer and the entire TEAM EEI staff is meeting your current needs?

Do you feel your trainer is sensitive and understanding of your disability and they meet your specific needs regarding you as an individual?

How do you feel the overall quality of services that TEAM EEI provides to you fits your needs?

How would you rate the level of communication that you have with the TEAM EEI trainer and staff that you are working with at this time?
